1. Prepare — start with structure
Deploy a framework into Incident and Actions, then refine the live form so reporters see the fields the operation actually needs. Health, manufacturing, and construction can each publish a form beside the default CAPA form. Installing a template does not create certification. It gives you a governed start.
2. Identify and report — capture the signal
Record facts, people, impact, severity, location, attachments, and what is known now. The first save does not have to be a finished investigation. Update later without losing that first save. AI rewrite is available on description so the record stays usable after the pressure passes. Capture-only mobile exists so the frontline is not waiting for a desk.
3. Assess and contain — decide on the same record
Score outcome and contributing factors. Preserve containment and root cause on the CAPA record, not in a side note. Confidential incidents stay visible to the reporter, the assigned owner, and administrators — not everyone with a licence.
4. Act — own the change
Create linked actions with an owner, due date, priority, and a source link back to the incident. Actions are not a second incident register. They are the accountable work layer: corrective, preventive, and follow-up. If the date slips, escalations should fire on that timeline, not wait for someone to remember a filter.
5. Recover and close — restore and verify
Effectiveness review and closure notes stay attached. Overdue work remains visible. Closure is the decision that the change held, not the moment the last comment was typed.
6. Learn — see the pattern
Executive packs and CAPA dashboards show recurrence, severity, and whether preventive action is landing. Duplicate detection helps stop the same event being typed twice so the register stays something leadership can trust. Print uses the filters already on the register: manager, area, date.
What “verified” means here
It does not mean a certificate appeared. It means a named person reviewed effectiveness on the same history as the event and the actions. If you cannot point at that review, you are not ready to call the incident closed — whatever the status field says.
